2008 AWARDS NOMINATION FORM
The National Philanthropy Day awards recognize individuals, organizations, foundations, businesses, and corporations for exceptional philanthropic and volunteer contributions in Colorado. The 2008 deadline for nominations is Friday, June 20, 2008 at 5:00 pm. For questions about the nomination process, please call Erin Binford at (303) 744-1688.

Completed nominations must be received by 5:00 p.m. Friday June 20, 2008.

The National Philanthropy Day online form is hosted by BlackTie Colorado. You will set up an account with an email address and password and will be able to attach your recommendation letters to the online form. You may want to download the Word document above before completing online.

NOTE: Each method of delivery must include nomination form, narrative information and letters of support. Separate delivery of any items will not be accepted.

The selection process will take place in late August and all nominators will be contacted by phone or mail by mid-September.

This year’s luncheon to honor the awardees will be held on Friday, November 14, 2008 at the Denver Renaissance Hotel.
